In order to obtain your Nationwide Sex Offender Registry Search, please review the information on the following pages and complete all required fields, including any former name used within the past 7 years. You will need to electronically sign a Disclosure form and Acknowledgment and Authorization form to authorize Hire Image LLC (www.hireimage.com), to conduct a background check of the Nationwide Sex Offender Registry on you. You will be sent a copy of your report to the email address provided upon completion of the search. The cost is $10 per name entered. If additional fees are incurred, you will be contacted as we do not retain credit card information.
